IMPORTANT NOTICE: HEALTH COVERAGE TAX DOCUMENTS (FORM 1095-C)
The Boyd Group is required to provide eligible employees with Form 1095-C, which contains important information regarding health coverage under the company's health plan for the previous calendar year.
What is Form 1095-C?
- Details about the health coverage offered to you, and if applicable, your dependents.
- Information about your enrollment, and if applicable, your dependents' enrollment, in the company's health plan.
Requesting Your 2024 Form 1095-C
The 2024 Form 1095-Cs have been prepared and are available upon request. If you were a full-time employee and were eligible for health coverage, you may request a copy of your form using the contact information below.
